This page provides helpful tips to access a fillable form, perform e-Submission uploads to submit information to MassHealth in a quick and efficient manner and view the status of your uploaded document(s).

Helpful Tips to perform a MassHealth e-Submission upload

To confirm you are permitted to perform the upload populate all the following fields and click Verify:

e-Submission Number — located on the notice you received from MassHealth
Head of Household Date of Birth
Medicaid ID — located on the MassHealth Card

Once your information has been verified you may be presented with a screen requesting you identify characteristics of people in your MassHealth household.

You should either indicate that ‘None of the above describes any member of my MassHealth household’
OR
Check off one or several of the other checkboxes that apply to your household
Then click Next

You will be presented with the screen containing links to the information requested in the MassHealth notice you received which referenced the e-Submission number you used in the verification step.

To access the fillable form, click on the link for the file format and language of the form you would like to fill out.

You will be presented with the form in another window for which you can type in your information.

Once you have completed filling out the application, save it to your device.

Please note:

You do not need to finish filling out the form in one session. You can save a partially completed form to your device, access it again when you have all the required information and save it again until it is complete. You should only choose the file to be uploaded after it is completely filled out.

You may also upload up to 9 additional files during the same e-Submission session as the one in which you uploaded your fillable form. Be sure to have all files you wish to upload prepared as once you leave the e-Submission application, you will not be able to upload additional files. Upon returning and using the same e-Submission number you will be navigated to the e-Submission Status screen to view the status of previously uploaded files.

How to upload your completed form:

Click Choose File, navigate to the file you have saved on your device, select it (if you have chosen the wrong file, you may either click on the trashcan icon to remove it or click Choose File and select the appropriate file) then click Next.

Select from the dropdown list to respond to any questions being displayed which may include:

Authorizing MassHealth to send your uploaded form to DTA for the purpose of applying for SNAP (only displayed if applicable)
Existing authorized representative (ARD) signing on behalf of the member

Enter your Electronic Signature for this upload in the text box next to the label.

Please note:

The name you type in the Electronic Signature text box should match the name exactly as it appears on the notice referencing the e-Submission number you used to access MassHealth e-Submission.

Click Upload Form to electronically submit your form for processing.

Note: You will receive a popup message asking you to confirm you would like to proceed with the upload of the fillable form and must click Yes or No.

Uploading Additional Files:

After successfully uploading your form, you will be presented with the File Upload Confirmation Page.

Select from the dropdown list to respond to the question regarding:

Do you have an additional file you would like to upload?
Note: a file can contain several documents

If you choose ‘Yes’, you will be navigated to the File Upload Portal.

Click Choose File, navigate to the file you have saved on your device, select it (if you have chosen the wrong file, you may either click on the trashcan icon to remove it or click Choose File and select the appropriate file), identify characteristics of the documents contained in the file you chose to upload, then click Upload.

Note: One or several of the checkboxes can be checked to describe documents in the file.

Acceptable file requirements:

Individual files must not be larger than 5MB (5 megabytes). All uploaded documents must be in one of the following formats:
  • Bitmap Image file (.bmp)
  • Graphical Interchange Format (.gif)
  • JPEG image (.jpeg)
  • Portable Document File Format (.pdf)
  • Portable Network Graphic (.png)

After uploading an additional file, you will be navigated to the File Upload Confirmation Page where you can choose to upload another file.

A maximum of 9 additional files can be uploaded during the same e-Submission session along with your fillable form. Once you leave the e-Submission application, you will not be able to upload additional files as when you return you will be navigated to the e-Submission Status screen to view the status of previously uploaded files.

Note: If you reach the limit of 9 files, you will only be allowed to click Next on the File Upload Confirmation Page which will navigate you to the e-Submission status screen.

If you choose ‘No’, you will be navigated to the e-Submission Status screen.

All areas denoted with a red asterisk (*) are required fields.

Status of Uploaded File(s)

After successfully uploading your file(s), you will be presented with the e-Submission Status screen.

The following fields will be displayed:

  • e-Submission number:
    e-Submission number — used for the first (initial) file uploaded using that number.
    Additional File # — used for each additional file that was uploaded after the initial file.
  • File Uploaded: The name of the file that was uploaded.
  • Upload Status: Provides confirmation that the uploaded file was Submitted.
  • Submission Date: Date the file was uploaded/submitted.
  • File Status: Status of each file that was uploaded.
    • Processed: MassHealth has reviewed the document(s)
    • Unprocessed: MassHealth received the document(s) and it will be assigned for review

Please note:

Initially the status of all uploaded files will be Unprocessed. After a file has been uploaded, you may return to the site at any point and after the verification step you will be presented with the e-Submission Status screen to check on any update to the status of the file(s).

Click the Go Back To Home button to return to the initial MassHealth e-Submission page.
